Care Coordinator (LMSW) - Preventative Medicine

Utilizes education, experience, medical social work skills and the Social Work Process to determine and facilitate the most appropriate level of care and identification of discharge needs. Works collaboratively with the team to identify discharge barriers and delays to optimize efficient use of acute care hospital days and prevent prolonged length of stay. Addresses all aspects of patient’s psycho
