What you'll do

• Provide outpatient primary care to patients ages two and older
• diagnose and manage acute and chronic conditions
• perform physical exams, preventive screenings, and wellness visits
• develop treatment plans and manage medications and follow-up
• educate patients and families
• coordinate with specialists, hospitals, and ancillary services
• maintain EMR documentation
• participate in quality improvement and collaborate with advanced practice providers, nursing, and support staff.
