What you'll do

• Coordinate and schedule daily district workloads
• supervise district nurses and home health aides
• review patient records and clinical content
• direct staff to community resources
• appraise staff performance and provide teaching and counseling
• review intake screenings for patient needs and home-care appropriateness
• assist with staff orientation
• see patients at home as needed
• participate in audits and interdisciplinary conferences
• review nurse schedules and submit daily rehospitalization referral/admission forms
• perform other duties as directed. May be asked to cover other supervisory staff when necessary.

Why this role
