What you'll do

• Coordinate and complete timely MDS assessments
• manage the RAI process and assessment calendar
• lead interdisciplinary care plan meetings
• ensure care plans reflect residents’ needs, goals, and services
• review clinical documentation for accuracy and regulatory compliance
• monitor changes in resident condition and ensure assessments are completed
• collaborate with nursing, therapy, social services, dietary, and other departments
• support accurate reimbursement
• stay current with CMS guidelines, Medicare requirements, and MDS updates
• and assist with audits, surveys, and quality-improvement initiatives.
