What you'll do

• Provide program information and support referral intake
• collect information and develop referrals according to agency procedures
• maintain hospice presence in hospitals, long-term care facilities, and other referral sources
• coordinate with providers, case managers, and staff involved in referrals and admissions
• assess patient eligibility and appropriateness
• communicate assessment findings to primary physicians and the interdisciplinary team
• support discharge planning and timely hospice admissions.
