Community Health Worker – Cardiovascular Health

Job overview

The Community Health Worker role is onsite in Dover, NJ. Pay is $22.12–$25.64 per hour. The position supports the Center for Justice in Cardiovascular Health at Zufall Health Center. The worker engages patients at risk for or diagnosed with cardiovascular disease, provides tailored health education and coaching, and helps coordinate appointments and community resources. By documenting needs and following up on referrals, the role supports care-plan adherence and coordinated, whole-person care.

What you'll do

• Identify and engage patients at risk for or diagnosed with cardiovascular disease
• provide culturally responsive education and lifestyle coaching
• assist with appointments, reminders, and transportation arrangements
• conduct social-determinants-of-health screenings
• document in eClinicalWorks
• refer patients to community resources and follow up
• collaborate with care teams and community organizations
• support quality improvement and program reporting.
