What you'll do

• Provide care management and coordinate direct care and support services
• assess mental health, substance use, risk, and health needs
• develop individualized service plans and crisis plans
• coordinate community services and transportation
• conduct outreach and groups
• support daily living skills and medication management
• maintain documentation
• assist with residence operations and guest activities.
