Registered Nurse (RN) Care Manager

What you'll do

• Travel to clients’ homes and complete nursing assessment and observation documentation
• evaluate clients and caregivers
• ensure client needs align with care plans
• coordinate quality-care procedures with management
• assess physical, mental, and psychosocial needs and develop multidisciplinary care plans
• mentor staff
• advocate for client rights
• address complaints with management
• coach staff on safety
• supervise in-home caregivers and verify skills and competency
• follow Virginia home care licensure regulations.
