What you'll do

• Evaluate patients and physician orders for home care eligibility
• conduct face-to-face transitions and educate patients
• present needs for branch approval
• document encounters
• coordinate transfer orders and ancillary services such as DME and infusion
• liaise with healthcare providers and referral sources
• coordinate resumption of care when applicable
• provide follow-up feedback on readmissions and non-admits
• perform sales administration duties.
