What you'll do

• Meet with physicians, hospital discharge planners, and other staff to promote hospice and generate appropriate referrals
• attend networking programs
• coordinate education opportunities
• track and report referral-source contacts
• participate in weekly team conferences
• assist with special events, health fairs, public relations, and admission documentation as assigned.
