What you'll do

• Provide direct in-person patient care during on-call, weekend, and after-hours shifts.
• Conduct initial and ongoing assessments of physical, emotional, psychosocial, and spiritual needs.
• Support the Interdisciplinary Group (IDG) process through chart reviews and care plan updates.
• Educate patients and families on disease progression and symptom management.
• Maintain accurate clinical documentation and support regulatory compliance.
