The Case Manager – Safe Options Support role is an onsite, field-based, full-time position in Buffalo, NY, paying $23–$26.62 per hour depending on education and experience. Endeavor Health Services is a not-for-profit organization focused on accessible behavioral health services and emotional wellness in Western New York and surrounding communities. The case manager provides outreach and person-centered support to people experiencing homelessness, helping address health and social needs and navigate the transition from street or shelter to housing. The role continues after housing placement to support stability, independence, and connection to community care; periodic on-call coverage and flexible hours are required.

What you'll do

• Conduct outreach and engagement in community locations, shelters, hospitals, and emergency departments
• assess health and social needs
• coordinate housing applications and move-in support with the SOS resource hub
• participate in hospital discharge planning
• collect and report data
• work with participants and housing providers to address issues affecting housing stability
• connect participants with community services
• accompany participants to appointments and address care barriers
• complete psychosocial assessments and document progress
• attend team meetings and supervision.

Why this role
