Registered Nurse — Director of Nursing, Home Care

What you'll do

• Conduct in-home assessments and reassessments, including after-hours and weekend visits when required
• develop and monitor care plans
• supervise CHHAs
• review documentation and conduct competency checks
• maintain regulatory compliance and clinical records
• lead the Joint Commission accreditation project and QAPI program
• support caregiver onboarding and referral relationships
• participate in shared on-call rotation.
