The role supports recovery and quality of life through treatment, monitoring, documentation, and coordination with patients, caregivers, and the interdisciplinary team.

What you'll do

• Provide physical therapy interventions under a physical therapist’s direction and a physician-approved plan of care
• observe, document, and report patient responses and condition changes
• document supervisory activities
• support wellness and prevention
• participate in care conferences, in-services, audits, and quality improvement activities
• coordinate follow-up care and community referrals
• maintain confidentiality.
