MDS Nurse Assistant (RN/LPN)

What you'll do

• Coordinate and gather data for resident assessment instruments
• implement and update individual care plans
• evaluate resident needs and level of care
• participate in managed-care pre-certifications and recertifications, care conferences, QAPI and committee meetings
• obtain and help analyze CASPER reports
• support timely completion of assessments and staff education
• participate in policy review and surveys
• maintain confidentiality
• coordinate with therapy on rehabilitation and assessments.
