What you'll do

• Develop and maintain referral relationships with physicians, discharge planners, social workers, elder law attorneys, financial advisors, and community organizations
• conduct outreach meetings, presentations, and industry-event participation
• understand client needs, coordinate initial assessments, and connect families with care management resources
• collaborate with internal teams
• document referral and outreach activity in the CRM
• work toward outreach and referral-generation goals.
