What you'll do

• Work with eligibility, business development, and enrollment teams to determine participant eligibility
• oversee home visits and clinic appointments for level-of-care assessments
• oversee referral and state enrollment documentation
• coordinate with Medicaid departments
• support outreach and census growth
• serve on the interdisciplinary team and coordinate further assessments
• participate in peer review and advise staff on medical and nursing issues
• supervise Eligibility RNs
• manage scheduling, productivity, performance reviews, hiring, training, and team development.

Why this role

Requires an associate degree from an accredited nursing school, a current registered nurse license in the state of work, current First Aid and BLS certifications before hire, 8+ years of healthcare experience with emphasis in geriatrics, and 1+ year working with frail or elderly people.
