What you'll do

• Meet participants in their homes at least every six months
• explain program rights, responsibilities, and risks
• assess and monitor participant or representative ability to participate
• assist with initial and updated service or spending plans
• monitor plan utilization
• respond to complaints and grievances and report required incidents
• document visits and interactions
• schedule visits and complete administrative tasks
• attend required training
• travel within an established region to meet participants.
