SNAP Case Manager

Job overview

The SNAP Case Manager role is onsite in Saint Augustine, FL, with a Monday–Friday schedule from 9:00 a.m. to 5:30 p.m. Pay is $21 per hour. Youth Crisis Center Inc’s SNAP program provides early intervention for children ages 6–11 who engage in aggressive or antisocial behavior and have encountered police. The case manager supports program delivery through intake, case management, therapeutic work, group facilitation, and administrative coordination. The role helps children and families participate in the evidence-based SNAP curriculum and supports program compliance and documentation.

What you'll do

• Assist with SNAP program startup and implementation of policies and standards
• conduct intakes and case management and provide therapeutic interventions as needed
• administer curriculum assessments and screenings
• monitor training delivery and effectiveness
• support contract compliance and timely documentation
• review client files
• inspect program areas
• facilitate groups and meet with individuals or families as needed
• participate in community activities and team meetings
• assist with client and family transportation as needed
• attend required training.
