What you'll do

• Provide 24/7 supervision and care in the provider’s home
• maintain a safe and clean living environment and ensure basic supplies are available
• assist with daily living activities
• administer prescribed medications as applicable under Kentucky MAP guidelines
• schedule and transport the individual to appointments
• monitor health and report concerns
• implement the Person-Centered Plan
• support independent living, socialization, communication, and behavioral self-regulation
• facilitate community participation and relationships
• use approved behavior supports and de-escalation techniques
• complete daily documentation and comply with agency and state requirements
• communicate with case managers, guardians, family, and agency staff
• attend team meetings and training.
