What you'll do

• Provide screening, preventive primary care, and medical care in members’ homes, community settings, nursing facilities, and pop-up clinics
• coordinate care for adult, women’s health, pediatric, and geriatric populations
• perform comprehensive evaluations and address acute and chronic primary care concerns
• establish diagnoses and care plans
• order or perform appropriate diagnostic testing
• coordinate post-discharge care
• document in the electronic medical record
• participate in alternating on-call coverage
• travel locally, including driving up to 120 miles a day regularly within the assigned region.
