It helps residents transition safely to their next living environment by coordinating follow-up services and equipment as needed.

What you'll do

  • Meet residents’ medically related emotional and social needs
  • complete and maintain grievance and reportable processes
  • conduct care conferences, 72-hour meetings, MDS assessments, and individualized care plans
  • coordinate resident discharge planning, including doctor discharge visits, home health, PT, OT, and durable medical equipment for safe return to a new living environment.
