Job overview
The Qualified Mental Health Professional (QMHP/QMHP-T) role is based in Portsmouth, VA, with sessions conducted in clients’ homes or the community. Pay is $23.00–$25.00 per hour. Corners of Life Family Services provides community-centered mental health care to individuals and families. The role delivers assessment and therapeutic services, develops and monitors individualized plans, and coordinates with care partners to support clients’ mental health goals in accordance with Virginia and Medicaid requirements.
What you'll do
- Conduct mental health assessments and diagnostic evaluations
- develop, implement, and monitor individualized treatment and service plans
- provide individual, group, and family counseling
- coordinate care with multidisciplinary teams, case managers, and community partners
- maintain timely clinical documentation
- monitor progress and adjust plans as clinically appropriate
- follow Virginia DBHDS regulations and Medicaid guidelines.
