Practice Transformation Specialist, Maryland/Delaware

What you'll do

• Implement products and ACO initiatives with market leadership
• support wellness visits, transitions of care, and active management
• help practices meet goals in transformation plans
• assist with training office staff, physicians, and providers on processes and technologies
• assess training needs
• build relationships with providers and healthcare executives
• relay practice concerns and improvement opportunities
• travel up to 25% for local practice visits.
