What you'll do

• Visit patients in their homes for one-on-one visits
• complete initial and ongoing assessments of physical, psychosocial, and environmental needs
• educate patients and families on home care strategies and procedures
• recommend devices or tools that may improve quality of life
• communicate with clients, families, team members, and other health professionals
• assist with training new employees and administrative compliance activities.
