What you'll do

• Supervise, coach, and support nurses and assigned clinical staff
• review patient assessments, plans of care, visit schedules, and documentation
• coordinate with physicians, therapists, social workers, aides, and other team members
• support home health admissions, resumption of care, recertification, and discharge
• support hospice admissions, symptom management, care conferences, and end-of-life coordination
• participate in interdisciplinary meetings
• monitor quality and support staff education
• ensure practices and documentation align with agency policies and requirements
• provide clinical guidance and assist with patient care when needed.
