Insurance Navigator – Medicaid Redetermination

What you'll do

• Serve as a primary contact for Medicaid redetermination and help complete and submit renewal applications
• identify patients at risk of losing coverage and conduct outreach
• assist with forms, documentation, and state eligibility systems
• provide enrollment assistance for Medicaid, CHIP, and ACA Marketplace plans
• explain coverage transitions, benefits, costs, and provider networks
• advocate with Medicaid offices, managed care organizations, and insurers when appropriate
• coordinate with care teams and community organizations
• document interactions and outcomes in the EHR and eligibility systems
• track retention and renewal measures and support compliance, audits, and quality improvement.
