What you'll do

• Provide digital health services, including virtual care and remote therapeutic monitoring.
• Participate in the development of the digital health program.
• Serve as a liaison between patients and in-clinic treating providers.
• Deliver telehealth visits including evaluation, assessment, and treatment plan development.
• Perform Telehealth and Remote Therapeutic Monitoring (RTM) services.
• Communicate with primary treating clinicians.
• Maintain reporting and data analysis of regional service metrics.

What we're looking for

Education

Diploma from a CAPTE-approved Physical Therapy Program

Experience

Minimum 2 years of relevant clinical experience (outpatient)
