Hospice Team Physician

On-call coverage is part of the role to support continuous physician availability.

What you'll do

• Provide clinical leadership within the Interdisciplinary Group
• guide hospice patient care, symptom management, and medication review
• develop and update individualized plans of care
• complete certifications, recertifications, and face-to-face attestations
• collaborate with attending physicians, providers, patients, and families
• participate in on-call coverage
• support hospice admissions, recertifications, revocations, and discharges
• maintain compliant medical documentation.
