What you'll do

• Complete OASIS assessments, including Start of Care, Resumption of Care, recertifications, and significant-change visits. Work with physicians to build and update care plans
• provide skilled nursing services, treatments, and preventive care within RN scope
• monitor and report changes
• teach patients and families
• supervise LPNs and home health aides as assigned
• document visits
• and participate in scheduled on-call or after-hours coverage.
