What you'll do

• Promote collaboration across the patient-centered care team
• direct and support coordination involving providers and clinical staff
• co-manage high-risk patients through assessments and care plans, including home and hospital visits when required
• educate patients and caregivers
• provide case management and advocacy
• maintain communication to support adherence and self-management
• liaise regarding medical and dental benefits
• document progress and barriers
• protect confidential information and support service improvement reporting.
