What you'll do

• Use the nursing process to assist patients and caregivers with scheduling
• advocate for patients and families
• implement discharge plans and coordinate reminders and follow-up visits
• ensure warm handoffs of patient and clinical context
• liaise with ambulatory clinics, free-standing emergency departments, and virtual services
• coordinate care transitions using clinical judgment
• educate patients and families about after-visit summaries
• identify patient problems and use resources
• document orders, care plans, and interactions in the EMR.
