Referral Intake Coordinator

What you'll do

• Receive, review, and process referrals
• collect accurate intake data and hand off referrals to clinical teams
• respond to calls and inquiries and help match individuals with services
• gather client health information and maintain it in the EMR
• share referral-partner updates with sales staff
• assist home and community-based services staff with care coordination and client service.
