What you'll do

• Complete admissions, recertifications, supervisory visits, and other nursing visits
• assess medical, functional, psychosocial, and environmental needs
• develop and update plans of care
• supervise and coach Home Health Aides
• monitor patient conditions and report significant changes
• educate patients and caregivers
• complete clinical documentation
• coordinate with physicians, caregivers, office staff, and interdisciplinary team members
• participate in chart reviews, quality improvement, compliance activities, meetings, and training.
