Hospice Registered Nurse (RN) Case Manager

What you'll do

• Coordinate and oversee patients’ home care
• collaborate with care teams to develop and implement care plans
• educate families about patient condition, needs, and care
• perform physical assessments and check vital signs
• manage medications and catheters
• complete accurate, timely documentation
• supervise home health aides
• communicate patient progress to the clinical manager, physician, and care team.

Why this role

Requires a current Registered Nurse state license, driver’s license, and CPR certification. Two years of registered nursing experience and one year of full-time hospice experience are preferred.
