The Registered Nurse (RN) – Home Health – PRN Weekends role is field-based, caring for patients in their homes within an assigned territory in Altus, OK. Compensation is not specified. What you'll do
- Conduct comprehensive in-home assessments
- develop and manage individualized care plans with physicians and care teams
- deliver skilled nursing care, medication management, treatments, and interventions
- coordinate care across therapists, aides, and physicians
- monitor progress and respond to changing conditions
- educate patients and families
- coordinate transitions and discharge planning
- maintain timely EMR documentation
- supervise and support Home Health Aides as needed
- participate in on-call rotation as appropriate.
