Registered Nurse (RN) Care Manager

The Registered Nurse (RN) Care Manager role is onsite in West Covina, CA, with care delivered primarily at the center and in participants’ homes as needed. Compensation is $56–$69 per hour, with a target annual bonus of 5%. Habitat Health’s PACE program coordinates medical and social care to help older adults live well in their homes and communities. The RN provides longitudinal nursing and case management for PACE participants, coordinating personalized care across the interdisciplinary team, center, home, and transitions between care settings.

What you'll do

• Conduct face-to-face physical, psychosocial, and behavioral assessments
• implement and document care-plan interventions
• administer medications and therapeutic interventions
• coordinate medication management, equipment, supplies, referrals, and transitions
• triage outpatient needs and escalate care with providers
• educate participants, caregivers, and team members
• provide wound care, IV therapies, and other procedures within RN scope
• delegate tasks to medical assistants and licensed vocational nurses
• participate in interdisciplinary team meetings.
