What you'll do

• Provides professional nursing care for residents of all ages in long term care under the supervision of a registered nurse, advanced practice provider, or physician.
• Demonstrates knowledge of nursing principles and practices within the full scope of nursing expertise.
• Utilizes appropriate age and population specific standards.
• Performs testing to evaluate resident status and provides necessary care.
• Recognizes problems, recommends solutions, and responds effectively in emergent situations.
• Ensures health, comfort, and safety of residents.
• Contributes to resident assessments and administers medications or treatments as ordered.
• Provides technical support to healthcare professionals.
• Documents resident interactions and outcomes.
• Establishes and maintains effective working relationships.
• Works effectively as a member of an interdisciplinary care team.
• Communicates clearly and maintains confidentiality.
• Counsels residents, family members, or advocates on prevention and treatment plans.
• Develops self-care programs.
• Performs other related duties as assigned.

What we're looking for

Education

Graduate from an accredited practical nursing program.

Experience

Minimum of 400 hours of experience as LPN at another facility or completion of a pre-licensure nursing program with supervised clinical experience across the life span.
