The Home Health Registered Nurse — South Territory role is onsite in Blue Island, IL, serving patients in their homes across the South Territory. Pay is $60–$120 per hour. Anuco Home Health Care is a Medicare-certified agency supporting recovery, safety, independence, and quality of life through in-home care. The nurse assesses patients, develops and coordinates individualized care plans, provides skilled nursing services, and helps patients manage their health safely at home. The role also coordinates with the interdisciplinary team, educates patients and caregivers, supervises assigned clinical personnel, and maintains timely, accurate documentation.

What you'll do

• Complete comprehensive nursing and OASIS assessments
• develop, implement, evaluate, and revise care plans
• provide skilled procedures, treatments, medication management, education, and preventive or rehabilitative care. Coordinate with physicians and interdisciplinary providers, supervise assigned LPNs and Home Health Aides, document in Axxess, and provide on-call, weekend, or holiday coverage when assigned.
