The Hospice Social Worker (LSW) - PRN is an onsite role based in Marseilles, IL. , though the range is $31.00 - $38.00 per hour, supporting the Transitions Hospice interdisciplinary team in providing essential psychosocial care to patients and families. This role exists to serve as a trusted advocate and counselor throughout the hospice journey. The Social Worker conducts psychosocial assessments, connects families with community resources, and provides ongoing emotional support to ensure holistic care for patients and their caregivers.

What you'll do

• Conduct psychosocial, spiritual, and pre-bereavement assessments
• provide ongoing emotional support and counseling
• identify and connect families with community resources
• guide families through Advanced Directives
• assist with facility placement
• maintain clinical documentation
• participate in interdisciplinary team meetings
• manage caseload compliance.
