Per Diem Home Health Registered Nurse (RN) - Psych is an onsite, per-diem role in Vero Beach, FL, paying $55–$130 per hour. The nurse supports Dedicated Home Health’s Medicare Home Health team, providing skilled nursing care in patients’ homes. The role assesses patients, coordinates care with physicians and the interdisciplinary team, and supports recovery, chronic-condition management, and independence at home. It requires psych experience and hands-on OASIS assessment experience, with Medicare home health experience strongly preferred.

What you'll do

• Provide skilled nursing visits in patients’ homes
• complete OASIS assessments and Start of Care, Resumption of Care, Recertification, and Discharge visits
• reconcile medications and educate patients and caregivers
• manage disease processes and chronic conditions
• assess and treat wounds when ordered
• monitor changes in condition and communicate significant findings
• coordinate and update plans of care
• complete timely clinical documentation.
