The Home Health Registered Nurse, West Territory role is an onsite field position serving Chicago, IL, and surrounding communities, with patient visits in the West Territory. Compensation is $60–$120 per hour. Anuco Home Health Care is a Medicare-certified agency providing in-home care to support patients’ recovery, safety, independence, and quality of life. The nurse assesses patients, develops and coordinates individualized plans of care, and provides skilled nursing services in patients’ homes. The role supports safe health management at home through clinical care, patient and caregiver education, interdisciplinary coordination, and timely documentation.
What you'll do
- Complete initial and ongoing nursing assessments and assigned OASIS assessments
- develop, implement, evaluate, and revise individualized care plans
- provide skilled nursing procedures, treatments, medication management, disease education, and preventive or rehabilitative care
- evaluate response to treatment
- process physician orders
- communicate changes in condition to physicians and the clinical team
- coordinate with providers and caregivers
- educate patients and caregivers
- complete required documentation in Axxess
- supervise assigned LPNs and Home Health Aides
- participate in case conferences, meetings, reviews, QAPI activities, and training
- provide on-call, weekend, or holiday coverage when assigned.

Why this role
