Case Manager — Targeted Case Management

What you'll do

• Develop and implement treatment plans based on assessments
• coordinate access to medical, vocational, social, educational, financial, legal, housing, and chemical dependency services
• develop Individual Community Support Plans
• schedule face-to-face visits
• make culturally sensitive referrals
• participate in commitment processes and required reporting
• support independent living
• coordinate emergency plans
• assist with housing
• attend reviews and treatment meetings
• document services
• and transport clients to appointments.
