The Homecare Services Coordinator (Human Services & Care Coordination) role is hybrid in Minnetonka, Minnesota, with the listing located in Hopkins. Starting pay is $24–$27 per hour. Asura supports people with disabilities and older adults through Minnesota’s Community First Services and Supports program. The coordinator develops and manages participant-centered service delivery plans based on county assessments, working with participants and service providers. Ongoing communication and knowledge of program and policy changes help participants access and maintain the goods and services they need to pursue greater independence.

What you'll do

• Develop and manage service delivery plans with participants based on county assessments under the CFSS program
• coordinate plan fulfillment through agency or budget providers
• communicate with participants, service agencies, lead agency providers, and DHS
• serve as a resource for assigned participants
• stay informed about relevant Minnesota, county, MCO, and service-agency policies and changes
• help participants receive and maintain required goods and services.
