What you'll do

• Respond to after-hours patient needs through telephone triage and in-person visits as appropriate
• complete nursing assessments in homes and care facilities
• provide clinical guidance and education
• complete admissions when necessary
• maintain clinical documentation
• and coordinate with the interdisciplinary care team. The posting lists an on-call schedule from Friday at 5:00 p.m. through Monday at 7:55 a.m., or Monday-to-Monday or partial days as needed, with up to two weekends per month.
