The Foster Care Case Worker, Unaccompanied Refugee Minors role is onsite in Rochester, NY, with a Monday-to-Friday, 9:00 AM–5:00 PM schedule. The case worker supports unaccompanied refugee minor foster youth and their permanency goals through case planning, counseling, service coordination, and culturally informed support. The role also maintains required records and provides scheduled after-hours Lifeline coverage.

What you'll do

• Develop treatment plans with foster parents, DHS, and youth as appropriate
• conduct required home and face-to-face visits
• coordinate medical, educational, vocational, and mental health services
• provide culturally informed casework
• attend court appointments
• maintain case records and reports
• support youth connections to cultural resources
• and cover Lifeline calls during a scheduled one-week rotation from 5 PM to 9 AM.
