What you'll do

• Assess patients and develop individualized goals and interventions

• support patients and families
• collaborate with the hospice team and participate in interdisciplinary meetings
• educate caregivers
• document assessments, interventions, responses, and progress
• maintain confidentiality and comply with applicable requirements.
