What you'll do

• Conduct nutrition assessments for patients at nutritional risk or as required
• implement evidence-based nutrition interventions
• evaluate specialized nutrition support needs and make recommendations
• document assessments in the medical record
• reassess and adjust nutrition care
• develop and direct nutrition and hydration plans for residents
• evaluate weight changes, admissions, wounds, standards of care, and menus
• participate in care conferences, meal rounds, survey readiness, and quality improvement.
