Community Health Worker — GROW Chronic Disease Support

What you'll do

• Conduct social-needs assessments and screenings
• coordinate appointments, referrals, and follow-up
• provide approved chronic disease education and self-management support within the CHW scope
• conduct approved home visits and community outreach
• track referrals and outcomes
• communicate field concerns to the clinical support team
• document contacts and support program monitoring and quality improvement.
