What you'll do

• Conduct initial and ongoing in-home client assessments
• develop and update care plans with clients, families, and care staff
• supervise home health aides and caregivers
• train staff on clinical skills, safety, infection control, and agency procedures
• audit caregiver documentation
• serve as a clinical resource
• participate in quality assurance and policy development
• maintain records under Minnesota regulations and HIPAA
• coordinate with physicians and other providers.
